• For "A" protocols (investigator-initiated), please submit GCRC Application Form, Ancillary Form and Enrollment Form.
• For "B" protocols (research/routine), please submit GCRC Application Form, Ancillary Form and Enrollment Form.
• For "D" protocols (sponsored protocols), please submit GCRC Application Form, Ancillary Form, Enrollment Form and D Financial Information form. The rates for "D" protocols can be found here: D Studies Rate
